We welcome requests for service or funds. We prefer at least 60 days advanced notice prior to your event or due date so that we can bring the request in front of our Oak Creek Lions Club Board. Our Board meets once a month on the 3rd Monday of the month.

Please provide as much detail as you can for us to consider. Include details such as:

- Who are you serving

- What the focus area is (health issues, people with special needs or disabilities, etc.)

- Where the funds will go

- How many people will benefit

- Others you may have asked to help and how
